QMB Boosters will not be responsible for those who order the wrong sizes. The Softshell jacket will be a signature item for our band members. Please understand the BLACK jacket is for the students and the GREY is for parents and staff. Jackets will be made available to try on prior to ordering. Before turning in your order form please look at the checklist below. Expected delivery date will be around the first QMB competition.

•Check your order form to be sure you are ordering your desired items and the correct quantity.
•Check to make sure you ordered the correct sizes.
•Did you include the upcharge for items xxl and higher?
•If ordering the softshell jacket, did you fill out all of the necessary information correctly?
•If ordering the NEW "Property of QMB" line did you include the necessary information?
•Did you fill out the order summary correctly?
This order form and payment is due to Mr. Biondo or the Apparel Box no later than Tuesday, August 29th following rehearsal.

*Performance wick short sleeved tee-shirt.
*Augusta Gear Bag.
*Hoodie or crew neck sweatshirt for the cool nights.

*Softshell Jacket...its personalized and its cool. (Feel free to try one on. They are in a box in the OPHS Band Room)
*Pajama Pants or sweatpants.
*Pompom beanie for the cold fall nights.
For Parents:
